Job Summary * Reports to the Social Work Manager. In partnership with nurses in the Care Management Department, social workers' focus is to assess and provide support to patients and families in the ED. Completes psychosocial assessments which enable them to address a range of issues including adjustment to illness, safety and protection, socioeconomic issues, non-compliance, cultural and spiritual issues, crisis intervention and end-of-life care, and facilitating appropriate discharge planning and disposition.
